アプリケーション入力（9ページ目：Health declarations／健康に関する申告）

【質問内容】
・In the last five years, has any applicant visited, or lived, outside their country of passport, for more than 3 consecutive months? Do not include time spent in Australia.
→ビザ申請日から５年遡って、申請者は自身のパスポート国の外に、３か月以上連続して訪問・住んでいたことがありますか？（オーストラリアを除く）

・Does any applicant intend to enter a hospital or a health care facility (including nursing homes) while in Australia?
→申請者はオーストラリアに滞在中、病院やヘルスケア機関（老人ホーム含む）に入る意図/予定がありますか？

・Does any applicant intend to work as, or study or train to be, a health care worker or work within a health care facility while in Australia?
→申請者はオーストラリア滞在中に、ヘルスケアワーカーとして働く/就学・研修する、またはヘルスケア施設内で働く意図/予定がありますか？
※以前は「doctor, dentist, nurse or paramedic（医師・歯科医・看護師・救急救命士）」という職種名での設問でしたが、現在は「health care worker（ヘルスケアワーカー全般）」という表現に変更されています。

・Does any applicant intend to work, study or train within aged care or disability care while in Australia?
→申請者はオーストラリア滞在中に、高齢者ケア（aged care）または障害者ケア（disability care）の分野で働く/就学・研修する意図/予定がありますか？
※こちらは新しく追加された設問です。

・Does any applicant intend to work or be a trainee at a child care centre (including preschools and creches) while in Australia?
→申請者はオーストラリア滞在中にチャイルドケアセンター（プリスクール、保育所を含む）で仕事をする、もしくは研修生となる意図している/予定がありますか？

・Does any applicant intend to be in a classroom situation for more than 3 months (eg. as either a student, teacher, lecturer or observer)?
→申請者はオーストラリア滞在中に3ヶ月以上学校へ通う意図/予定がありますか？（生徒、教師、講師、見学者などとして）

・Has any applicant:
ever had, or currently have, tuberculosis?
been in close contact with a family member that has active tuberculosis?
ever had a chest x-ray which showed an abnormality?

・During their proposed visit to Australia, does any applicant expect to incur medical costs, or require treatment or medical follow up for:

・Does any applicant require ongoing medical care or need special equipment, assistive technology or assistance from others for daily living?
→申請者のいずれかで、継続的な医療ケア、または特別な器具・支援技術（assistive technology）・他者からの日常生活の介助が必要な方はいらっしゃいますか？
※以前は「健康上の理由で移動やケアの介助が必要か（require assistance with mobility or care due to a medical condition）」という設問でしたが、現在は継続的医療ケア・日常生活支援を含むより広い表現に変更されています。

 

■過去5年間で自身のパスポートの国以外に3ヵ月連続して訪問・住んでいたことがある方（オーストラリアを除く）
上記設問で「Yes」を選択すると、「Visits to other countries」という詳細入力画面が表示され、対象期間中に住んだ、もしくは訪れたすべての国の詳細を入力します。

・Name（対象となる申請者を選択）
・Country（住んだ or 訪れた国名）
・Date from（入国日）
・Date to（出国日）
※入国・出国日は日付不明の場合は年度と月のみでもOKです。

入力したら’Confirm’をクリックして確定させます。
※同じ手順を繰り返し、全ての国を入力します。

※なお、「病院やヘルスケア機関に入る予定があるか」の設問で「Yes」を選択した場合も、同様に詳細入力画面（Intention to enter hospitals or health care facilities）が表示され、対象者（Name）・理由（Reason：Give birth＝出産／Training＝研修／Treatment＝治療／Visiting＝見舞い・訪問／Work＝就労から選択）・詳細（Give details）を入力してConfirmします。通常のワーホリ渡航ではいずれの設問も「No」となる方がほとんどです。

アプリケーション入力（10ページ目：Character declarations／人格に関する申告）

申請者がいずれかの項目に「Yes」と回答した場合、関連するすべての詳細を提供する必要があります。 複合申請の場合、適用される申請者を明記してください。

問題が刑事上の有罪判決に関連する場合、以下を提供する必要があります。
・犯罪の日付と性質
・判決のすべての詳細
・投獄またはその他の拘禁期間の日付

【質問内容】
・Has any applicant ever been charged with any offence that is currently awaiting legal action?
→現在訴訟中の、何らかの犯罪についての告訴を受けたことがありますか？

・Has any applicant ever been convicted of an offence in any country (including any conviction which is now removed from official records)?
→いずれかの国で、有罪判決を受けたことはありますか？（現在は公式の記録から取り除かれているものも含める）

・Has any applicant ever been the subject of a domestic violence or family violence order, or any other order, of a tribunal or court or other similar authority, for the personal protection of another person?
→申請者は、他者の保護を目的とした、DV（家庭内暴力）・家族間暴力に関する命令、または裁判所・審判所等によるその他の保護命令の対象となったことがありますか？
※こちらは新しく追加された設問です。

・Has any applicant ever been the subject of an arrest warrant or Interpol notice？
→逮捕令状を出されたり、国際警察の公示対象になったことはありますか？

・Has any applicant ever been found guilty of a sexually based offence involving a child (including where no conviction was recorded)？
→小児性犯罪に関する有罪が認められたことはありますか（有罪判決が記録されていないものも含める）？

・Has any applicant ever been named on a sex offender register?
→性犯罪者として登録されたことはありますか？

・Has any applicant ever been acquitted of any offence on the grounds of unsoundness of mind or insanity?
→心神喪失や精神異常によって、犯罪が無罪となったことはありますか？

・Has any applicant ever been found by a court not fit to plead?

・Has any applicant ever had any outstanding debts to the Australian Government or any public authority in Australia?
→オーストラリア政府やオーストラリアの公共機関に未払いの負債があったことはありますか？

また自衛隊に勤められていた方はDischarge証明が通常求められます。（退職届、辞令書）、そちらの原本と英訳されたデータを
添付を最後に求められますので、自衛隊勤務経験ある方は事前に書類のご用意をしておくとスムーズです。

全項目にチェック後、Nextボタンをクリックして次に進みます。

アプリケーション入力（11ページ目：Working holiday declarations／ワーキングホリデーに関する申告）

画面冒頭に「Giving false or misleading information is a serious offence.（虚偽または誤解を招く情報を提供することは重大な犯罪です）」という警告が表示されます。

申請者は以下のことを宣誓します（各項目でYesを選択します）；　

・Understand that they must abide by the conditions of the visa.
→ビザの条件を遵守しなければならないことを理解しています。

・Understand that the visa they are applying for does not permit them to be employed in Australia with one employer for more than 6 months without prior permission.
→申請しているビザは、事前許可なく1つの雇用主に6ヶ月以上雇われることを許可していないことを理解しています。

・Understand that the visa they are applying for does not permit them to undertake studies or training for more than 4 months.
→申請しているビザでは、勉強やトレーニングを４ヶ月以上できないという制限があることを理解しています。

・Have sufficient funds for the initial period of their stay in Australia and for the fare to their intended overseas destination on leaving Australia.
→私はオーストラリアでの滞在期間の滞在費と、オーストラリアから次の滞在国へ発つための航空券代のための十分な資金を持っています。

・Understand that any employment is incidental to their holiday in Australia and the purpose of working is to supplement their holiday funds.
→オーストラリアでの就労はあくまで休暇に付随的なものであり、就労の目的は休暇資金を補うためであることを理解しています。
※こちらは新しく追加された宣誓項目です。ワーキングホリデービザは「休暇」が主目的で、就労はそれに付随するもの、という位置づけが改めて確認されるようになりました。

全ての項目にYesを選択後、Nextをクリックし次に進みます。

アプリケーション入力（12ページ目：Declarations／最終宣誓）

画面冒頭に「Giving false or misleading information is a serious offence.（虚偽または誤解を招く情報を提供することは重大な犯罪です）」という警告が表示されます。

申請者は以下のことを宣誓します（各項目でYesを選択します）；　

・Have read and understood the information provided to them in this application.
→この申請で提供された情報を読み、理解しました。

・Have provided complete and correct information in every detail on this form, and on any attachments to it.
→このフォームそして添付資料における全ての内容は、正確で最新の情報を提供しました。

・Understand that if any fraudulent documents or false or misleading information has been provided with this application, or if any of the applicants fail to satisfy the Minister of their identity, the application may be refused and the applicant(s), and any member of their family unit, may become unable to be granted a visa for a specified period of time.
→不正や語弊のあるドキュメント、情報などが発見された場合、または人物証明に関して条件を満たすことができなかった場合、この申請は拒否される可能性があり、申請者とその家族メンバーは一定期間ビザ取得資格を失うかもしれないということを理解しています。

・Understand that if documents are found to be fraudulent or information to be incorrect after the grant of a visa, the visa may subsequently be cancelled.
→ビザが発給された後に、情報の誤りや詐称が発見された場合は、ビザがキャンセルされる場合があることを理解しています。

・Understand that if this application is approved, any person not included in this application will not have automatic right of entry to Australia.
→ビザが発給された後、このビザに含まれていない人物がオーストラリア入国権利を自動的に得ることがないということを理解しています。

・Will inform the Department in writing immediately as they become aware of a change in circumstances (including change of address) or if there is any change relating to information they have provided in or with this application, while it is being considered.
→もし、環境の変化に伴う情報の変化（住所の変更も含む）があった場合、または提供した情報の審査中に、その誤りに気づいたりした場合、直ちに書面で移民局に報告する義務があることを理解しています。

・Have read the information contained in the Privacy Notice (Form 1442i).
→プライバシーに関する取り扱いを含む情報（フォーム1442i）を読みました。

・Understand that the department may collect, use and disclose the applicant’s personal information (including biometric information and other sensitive information) as outlined in the Privacy Notice (Form 1442i).
→移民局は、プライバシーに関する取り扱い（フォーム1442i）に記載されている通り、申請者の個人情報（生体情報やその他慎重に扱うべき情報を含む）を集め、使い、開示する可能性があることを理解しています。

・Give consent to the collection of their fingerprints and facial image if required.
→必要に応じて、申請者の指紋や顔画像を収集されることに同意します。

・Understand that, if required to provide their fingerprints and facial image, the applicant’s fingerprints and facial image and biographical information held by the Department may be given to Australian law enforcement agencies to help identify the applicant and determine eligibility for grant of the visa being applied for, and for law enforcement purposes.
→もし指紋や顔写真の提出を求められたら、移民局によって保持された申請者の指紋と顔写真生体情報は、申請者の身元確認や、申請中のビザの発給に適格かどうかの判断、また法的執行の目的で、オーストラリア法執行機関に渡されることを理解しています。

・Give consent to Australian law enforcement agencies disclosing the applicant’s biometric, biographical and criminal record information to the Department to help identify the applicant, to determine eligibility for grant of a visa and for law enforcement purposes.
→オーストラリアの法的執行機関が、申請者の身元確認、ビザ交付や法的執行に適格かどうかの判断をさせるため、申請者の生体情報、犯罪歴を含む個人情報をオーストラリア移民局に開示することに同意します。

・Give consent to the Department using the applicant’s biometric, biographical and criminal record information obtained for the purposes of the Migration Act 1958 or the Citizenship Act 2007.
→オーストラリア移民局がMigration Act 1958またはCitizenship Act 2007にのっとり、保持してる申請者の生体情報や犯罪歴を含む個人情報を使用することに同意します。

As an applicant（申請者として）
・I understand that if my visa ceases to be in effect and I do not hold another visa to remain in Australia at that time, I will be an unlawful non-citizen under the Migration Act 1958. As such, I will be expected to depart from Australia, and be subject to removal under the Migration Act 1958.
→ビザが失効し、その時点でオーストラリアに滞在するための他のビザを保持していない場合、私はMigration Act 1958のもとで不法滞在者（unlawful non-citizen）となることを理解しています。その場合、オーストラリアから出国することが求められ、同法に基づき退去強制（removal）の対象となります。
※こちらは新しく追加された宣誓項目です。オーバーステイ（ビザの期限超過滞在）を防ぐための確認として加えられています。

Australian values（オーストラリアの価値観）
・Each applicant who is 18 years or over has read, or had explained to them, information provided by the Australian Government on Australian society and values, and agrees to the Australian values statement.
→この申請に含まれる18歳以上の全てのメンバーは、オーストラリア政府の提供するオーストラリア社会と価値観に関する情報を読み、または説明を受け、the Australian values statementに同意します。
※画面には参照リンクとして「Life in Australia booklet（オーストラリアでの生活ブックレット）」と「Australian values statement（オーストラリアの価値観に関する宣言）」が表示されます。これらの内容を確認したうえでYesを選択します。

 

以上でアプリケーションの入力は終了です！

全ての項目にYesを選択し、Nextより次に進みますと確認画面となります。
今までに入力した全ての情報が表示されますので、再度入念に確認してください。
確認後、Nextをクリックすると、以下の書類添付画面へと進みます。

アプリケーション入力（書類添付画面：Attach documents）

画面冒頭に次の案内が表示されます。
「下記の書類を添付してからNextをクリックし、申請を提出してください。すべての必要書類（required documents）を添付せずに提出する場合は、その理由の入力が必要になります。必要書類が揃っていない申請は、処理に時間がかかる場合があります。」
※書類のスキャンには品質・形式の要件（quality and formatting requirements）があります。鮮明で読み取れるスキャン・写真を用意してください。1申請あたり最大60ファイルまで添付可能です。

Required（必須書類）として、以下の2種類が表示されます。

■Travel Document（Document Type：Passport）：パスポートのスキャンまたは写真を添付

■Evidence of funds for stay in Australia and departure：残高証明書のスキャンまたは写真を添付

【書類添付方法】

①各書類の欄を開き、「Add documents」の下にある「ファイル選択（Choose Files）」をクリックして書類を選択します。Description（説明）欄は任意です。

②書類を選択後、ファイル名が表示されます。ファイル名を確認後、「Attach」をクリックし、書類の添付が完了します。

③残高証明書を添付する際は「Evidence of funds for stay in Australia and departure」の欄を開き、上記と同様に書類を添付します。

※添付が成功していれば、各項目が「１ Received」（添付前は「0 Received」）と表示されます。

必要書類の添付が完了したら、Nextをクリックして次に進み、Submit（申請確定）へと進みます。Submitをクリックすると申請が確定され、お支払い画面に進みます。

アプリケーション入力（お支払い画面：Payment）

現在の流れは、書類添付画面でSubmitした後、まず請求先住所（billing address）などの情報を入力し、その画面でもう一度Submit（送信）することで、はじめてクレジットカード情報の入力画面に進む、という2段階の構成になっています。

※上記は旧バージョンのお支払い画面です。参考としてご覧ください。

クレジットカード情報の入力画面では、おおむね以下の項目を入力します。
・Type of Credit card（カードの種類を選択）
・Credit Card Number（カード表面に記載された15または16桁の番号）
・Expiry Date（有効期限）：月／年度
・Name on Credit card（カード保持者）：クレジットカード上のお名前をローマ字で入力

*カードはご本人様でなく、第3者、友人や家族名義であっても問題ありません。

・申請料：840ドル（2026年7月1日改定・1回目のワーホリの場合）
※セカンド・サードワーキングホリデービザの場合は1,000ドルです。

カード決済手数料（目安・変更されることがあります）
American Express — 1.40%
Diners Club International — 1.99%
JCB — 1.40%
Visa/Masterカード — 1.40%
PayPal — 1.01%
※手数料率は改定されることがあります。最新の料率は必ず決済画面でご確認ください。

クレジットカード情報を入力後、決済を確定するボタン（Submit等）を押してください。

カード決済がうまくいきましたら、TRN番号（照会番号）が発行されますので、必ずメモしておいて下さい。
また、領収書、申請内容のＰＤＦファイルも、最終画面でダウンロードできます。念のため、そちらをプリントアウトして保管しておいてください。
